Personal Health Care Record: Personal Health Care Record (PHR) is the digital health-related information of an individual, which comply with nationally recognised standards. The most salient feature of the PHR is that it can be drawn from multiple sources all the while being managed, controlled, and shared by the consent of the particular individual. However, other stakeholders of the healthcare system like doctors will have access to view, edit, and update information in the individual’s PHR.

Electronic Medical Record: EMR is like a digital version of a patient’s medical history. It helps the healthcare stakeholders like clinics to check a patient’s medical and treatment history from just a SINGLE health facility. The record allows easy identification of the patients whose preventive screenings or checkups are due.

Digi Doctor Platform: Digi Doctor Platform is a single and frequently updated repository of all the doctors in the nation. It provides vital and basic information of the doctors, such as qualifications, specialisations, medical experience, registration number with the state council, etc.

Health Facility Registry:  This key element helps the public as well as private health facilities in the country to get registered. This allows seamless sharing of health data between them, offering the best healthcare services to the patients.

3. Who Is Eligible For Taking This Benefit?

Under the National Digital Health Mission, there is no prerequisite to get the National Digital Health Card. Every citizen of India is eligible for the unique health ID card. You just require some documents to register such as your basic information, mobile number, and Aadhar card (optional).
