1. Visit the YSR Navasakam website and click on the login tap in the top right corner.

2. Login to the website with your employee/pensioner number and password. The password will be the same as your employee/pensioner number.

3. Next, click on the “Aarogyasri Health Card” application form.

4. Fill out the application form with the required details and upload the supporting documents.

5. Last, submit the application form. Remember to take a printout of the same for your records.
