CLAIM FOR SECTION I

How to file a Claim
  • In case of any event leading to a claim under the policy, please call our Toll-free Number 1-600-226-226
  • Our Claims Service Representative will guide you on the claim procedures and documents required
  • Complete the claim form relevant to the nature of loss as indicated below.
  • Attach the documents mentioned against the claim type

Death Claims
  • Completed Claim Form;
  • FIR;
  • Post Mortem Report;
  • Final Report from police;
  • Copies of Inquest Report of criminal case filed by police (IF ANY);
  • Statements of Gram Pradhan / Municipal Councilor/Post Master/School Teacher
  • If Claimant is not the spouse, succession certificate from Collector’s Office.
Non Fatal Claims resulting in Permanent Total Disablement as covered in the policy
  • Completed Claim Form;
  • FIR/Report to Police Station;
  • Certificate regarding disability from a Government Hospital or a Doctor equivalent to CMO / Dy. CMO;
  • Statement of witnesses;
  • Original Admission / Discharge Card of hospital;
  • Investigation / Lab reports (x-ray, etc.).
Death Claims Due to Snake Bite
  • Completed Claim Form;
  • Post Mortem Report;
  • Statements From Neighbours;
  • Statement from Witness/Person who discovered the body;
  • Investigation Report (MAY BE CALLED FOR DEPENDING ON THE CASE );
  • If Claimant is not the spouse, succession certificate from Collector’s Office

  • Documents, in addition to those mentioned above maybe called for, depending on the nature of accident and claim lodged.
  • You may also send the claim form with annexures to our Claims Processing Cell at the following address :

  • A claim form will be forwarded to you by mail, email or fax.
  • Complete the claim form relevant to the nature of loss as indicated below.
  • Attach the documents mentioned against the claim type.

For Accidental Injury Claims
  • Claim form
  • Police Report, if accident is reported to Police
  • Medical papers, pathology reports, X-ray reports, as applicable
  • Doctor’s medical prescriptions, Itemized bills and cash memos *
  • Hospital Discharge Card
For Hospitalisation due to Illness/Disease
  • Claim Form
  • Medical papers, pathology reports, X-ray reports, as applicable
  • Doctor’s prescription and line of treatment suggested
  • Itemized bills and cash memos *
  • Hospital Discharge Card

* Copies of fully itemized medical bills. Itemized bills must show the patient’s name, date of treatment, the type of treatment given, the diagnosis or nature of condition being treated and the Hospital/Nursing Home’s name and address.

  • Documents, in addition to those mentioned above maybe called for, depending on the nature of claim lodged.
  • You may also send the claim form with annexures to our Claims Processing Cell at the following address :
